Thinking BIG: The Modified BIG score
|
Traumatic brain injury (TBI) remains a major healthcare burden, driving high costs and resource utilization. The Brain Injury Guidelines (BIG) offer a tiered approach to managing mild TBI with intracranial hemorrhage, stratifying patients by injury size and directing care from ED observation to ICU admission as needed. Early studies suggest BIG may safely reduce unnecessary neurosurgical consults, repeat CT scans, and hospitalizations—freeing critical resources while maintaining patient safety. The authors of this article recommend several modifications to the original BIG guidelines. Overall, the modifications can be categorized as changes to either increase patient safety or to allow for consistent, widespread utilization of the guidelines. It's Sepsis September!
|
Who knew, but September is Sepsis Awareness Month. Did you forget to celebrate? Whether you partied or not, it's a good reminder that sepsis, and its subsequent organ failure, is the leading cause of death among trauma patients during the post-resuscitation phase of care. Our nursing care significantly influences a patient's risk factors, and our timely interventions are vital for early sepsis recognition and management. The Sepsis Alliance has many resources available. Be a Sepsis Superhero!

Are TCAR and TEP the Same Thing?
|
Well, sort of. TEP is the organization, TCAR is the course. TEP stands for TCAR Education Programs, an independent nurse-founded and nurse-led organization based in Oregon, USA. The TCAR (Trauma Care After Resuscitation) course was first introduced in 2011, followed shortly by PCAR (Pediatric Care After Resuscitation). Since its establishment, TEP has trained nearly 100,000 nurses and allied health professionals across more than 900 facilities in all 50 states and several other countries.

The FDA Approves an Over-the-Counter, Cuffless BP Monitor
|
Can we say farewell to the good old stethoscope and sphygmomanometer? The FDA recently approved the first cuffless, over-the-counter blood pressure monitor, the Hilo wristband, made by Aktiia. Using optical sensors, the bracelet-like device offers continuous readings through a smartphone connection. Although it needs monthly calibration, the Hilo has shown accuracy across different skin tones and body positions. Already approved in several countries and with over 120,000 units sold, the Hilo is expected to launch in the U.S. in 2026. However, it's currently intended for home use only.
